WORKAROUND:

DETAILS:

This problem may occur when the system's cache is highly utilized, a logical drive is in critical state (1 DDD drive) and 2 types of ServeRAID Adapters are installed.
This problem has only been able to be replicated in development test when a drive goes DDD during an "add to free array space" LDM operation in NetWare v4.2 with IWSP6, a large amount of network activity, and two types of ServeRAID adapters installed in the system.

Note: It may occur with other operating systems if the conditions regarding highly utilized cache with a critical logical drive are met, but development has not been able to replicate in any other operating system environment besides NetWare.
