Abstract: MULTIPLE DDD DRIVES caused by INCORRECT CHANNEL MAPPING
SYMPTOM:
After one drive becomes DDD, one or more other drives will appear to be in a DDD drive state.

FIX:
NOTE: All further references to "Bank" or "Banks" represents those internal
to the System and / or those located in a System-connected External Enclosure.
Ensure the Channel-bank mapping correctly reflects the SCSI cabling of the ServeRAID /
ServerRAID II Adapter / Controller to the Hot-Swap backplane(s) as follows:
EXAMPLE:
Ch1 on the ServeRAID goes to Bank E
Ch2 on the ServeRAID goes to Bank D
Ch3 on the ServeRAID goes to Bank C
